This is the practice’s patient messaging program. How to opt in. SMS opt-in is collected only through an optional checkbox on a website form. The checkbox is unchecked by default and is not required in order to submit the form. Frequency. Message frequency varies based on your appointments and enquiries. Cost. Message and data rates may apply. We do not charge for these messages; your mobile carrier may. Opt out. Reply STOP to any message to stop receiving them. You will receive one confirmation message and no further messages. Help. Reply HELP for assistance, or contact the practice on (480) 282-8386 or through our contact page. Carriers. Carriers are not liable for delayed or undelivered messages. Consent. Consent to receive text messages is not a condition of purchase or treatment. Privacy. We do not share, sell, or provide your mobile phone number or messaging consent data to third parties or affiliates for marketing or promotional purposes.

For information about medical privacy rights and uses or disclosures of protected health information, please contact the practice. Emergencies. Text messages may not be encrypted. Do not use text messaging for emergencies or urgent medical needs; call 911 or contact the practice directly.
